For governance body , there are various forms to submit for attending consultant to filled up in order to process the claims. This claim is solely for a worker and their dependent which registered with the company. The claims will be processed by Perkeso/ Kumpulan Wang Simpanan (EPF) or any governance body itself. There are no agents or third parties shall be processed this claim for a patient except a patient himself or representative from Human Resource. For Socso/Perkeso , there are three (3) different claims form which depend on patient’s condition. The forms are as follows ;

1. Laporan Kemalangan ( Lampiran 1 ) – mostly for accident/industrial accidents related
2. Laporan Pencen Khidmat – mostly for occupational hazard
3. Laporan Pencen Ilat – for claims once insured not able to do routine jobs
