Reminder

If a patient has not visited BSC for more than 7 years from the date of application, please contact the Medical Records staff at extension 21101 / 11144 to verify the status of the patient’s record. This will help determine whether the record is still available or has been disposed of, ensuring a smooth submission process.

Request For A Medical Report

Effective in 2022, we no longer received any medical report submission through email and hardcopy claim forms for O&G report, MAR report, Accident claim, APS claim, lawyer report & specialist report, M.A.R and free format except for diagnosis, endorsement and copy of the medical report. Now we are using a medical report request via websites.

What Are Documents Required

Consent/Authorization Form
Patients’ written consent is compulsory for the release of medical information. For patients under 18 years of age, or has a mental incapacity to consent for the release of information, or are deceased, consent is required from the parents/guardian / next-of-kin / legal representative (You may download BSC consent for release information if not available) provided link :

Claim form
A complete claim form or cover letter need to be attached, provided space for doctor signature.

How Soon Can I Obtain My Medical Report

Medical Report or Insurance Claim is ready for collection estimated within 14 working days upon request exclude Saturday, Sunday and Public Holidays.

* There are cases where request may take longer than the stipulated completion time period. Delays in prompt processing may be due to:

  1. Unavailability of doctors (eg :away on leave)
  2. Patient has upcoming clinic appointments
  3. Patient has been admitted/hospitalized
  4. Patient has multiple reports requested from several doctors
  5. Visiting doctor without clinic session in Bagan Specialist Centre
  6. Doctor’s limited no. of sessions at Bagan Specialist Centre (e.g. only one sessions per week)

How Will I Receive The Completed Medical Report

Once your medical report complete, we will contact you to inform. Depends on your preferred way to collect.

    1. Collect the report from the medical report counter (MDF Room): 
      1. ID verification (NRIC or Passport) is required before collection
      2. Original receipt to be produced at point of collection
      3. If collecting on behalf of the requestor, an authorization letter signed by the patient is required
    2. Receive the report via email: 
      1. We will email the report to the email address provided in the websites.
    3. Receive the report via mail: 
      1. Report will be couriered to the address provided in the website
      2. We will courier to you by AIRPAK services and charge RM10 for postage
